- Connecting the participants with the relevant service providers
- Work with the formal and informal supports (family, friends, doctors, therapist etc.) in order to achieve your goal
- Assisting and teaching how to access the NDIS portal
- Monitoring and reviewing the progress
- Assisting with preparation for the next NDIS plan review meeting
